The violations included: storage of hazardous waste without a permit, failure to mark equipment subject to Subpart BB; failure to cap or double valve open ended valves and pipes; failure to conduct weekly inspections of hazardous waste storage areas; failure to properly manage containers of hazardous wastes: failure to maintain job descriptions for personnel handling hazardous wastes: failure to require annual training for personnel and failure to properly manage universal waste.
