An EPA contractor inspected the facility on June 9, 2016 and found the facility failed to have annual line tightness test or perform monthly monitoring on pressurizing piping, failed to provided adequate line leak detector for underground piping, failed to comply with financial responsibility requirements and failed to have adequate operation or maintenance of ground-water monitoring.
