1.Respondent failed to update their Process Hazard Analysis (PHA) to assure that the PHA is consistent with the current process. 2. Respondent failed to ascertain that each employee involved with an operating process has received and understood training required and has failed to prepare a record so stating, 3. Respondent failed to perform inspections and tests of process equipment consistent with applicable manufacturers' recommendations 4. Respondent failed to document each inspection and test that has been performed on the process equipment. 5. Respondent failed to correct deficiencies in equipment that are outside acceptable limits. The penalty was $5,800.00
