This Information Request and Administrative Compliance Order addresses CWA violations identified during a September 6, 2017 EPA inspection at the Respondent's WWTP. Violations include failure to develop and implement written Operations and Maintenance (O&M) procedures, and failure to perform inspections in accordance with the Mercury Minimization Program, as required by the WWTP?s SPDES permit. In addition, the Order requires the Village to eliminate or obtain SPDES permit coverage for unpermitted industrial stormwater discharges to the Cohocton River.
