Failure to select, design, and implement control measures to minimize pollutant discharges as required by the site specific storm water pollution prevention plan under Part 2.1.1 of the storm water general permit; 2. failure to include required documentation for the routine facility inspections in the report; and 3. failure of the facility's Storm Water Pollution Prevention Plan to be in compliance with Part 5.1 of their NPDES Multi-Sector General Permit.
