On October 30, 2018, EPA Region 6 issued a Consent Agreement and Final Order under its authority pursuant to Section 1423(c) of the Safe Drinking Water Act (Act), 42 U.S.C. Section 300h 2(c) to Flagship Oil, LLC located in Skiatook, Osage County Oklahoma. The CAFO was issued in response to violations of: 40 C.F.R. 147.2903(a) and 147.2912 by injecting fluids into an unauthorized injection well. The CAFO, US EPA Docket No. SDWA-06-2018-1103, proposes to assess an administrative penalty. Flagship Oil, LLC must respond to EPA Region 6 within 30 days of the date of the issued Complaint.
