On August 28, 2018, a Consent Agreement and Final Order (CAFO) was submitted to the Regional Hearing Clerk for the Dallas VA Medical Center for violations of RCRA hazardous waste management requirements. The violations, identified during an EPA inspection, included:
1) Operating without a permit or interim status
2) Failure to provide notification on non-receipt of termination manifests
3) Failure to complete hazardous waste manifests and ship hazardous waste with manifests, and
4) Failure to offer hazardous waste to a transporter with a EPA identification number.
The Compliance Order in the CAFO requires Dallas Veterans Affairs Medical Center to certify and document compliance and pay a penalty of $37,545.
