Administrative Order for several violations including: failure to monitor for total trihalomethane (TTHM) and haloacetic acid (HAA5) samples; failure to complete corrective action of a significant deficiency; failure to monitor for lead and copper; and failure to prepare and deliver annual CCRs and provide certification to the EPA.
