Respondent, which operates a concrete and asphalt generating facility, violated its NPDES permit by failing to properly monitor all pollutants, failing to perform some inspections, failing to properly document inspections, and failing to install and implement all appropriate control measures. The company agreed to pay a penalty of $24,500.
