On October 10, 2018, EPA Region 10 entered a consent agreement and final order to resolve Cascade Designs, Inc. violations of FIFRA. Cascade Designs, Inc. operates two pesticide producing establishments in Seattle, Washington. Cascade Designs, Inc. failed to file the Pesticide Report for Pesticide-Producing and Device-Producing Establishments (EPA Form 3540-16) for pesticides and pesticide devices produced in 2017 at both establishments by the reporting deadline of March 1, 2018. This is Cascade Designs, Inc.?s third such violation in the past five years. The company agreed to pay a penalty of $13,692.
