The Respondent incurred penalties and injunctive relief compliance requirements for violating its EPA Permit and the UIC regulations at 40 C.F.R. Part 144 by injecting in its Riverside #2 well (Location: 42.94486111, -85.73819444) fluids from unapproved sources, failing to obtain EPA's prior approval to making changes to its injection fluids, failing to submit an analysis of the new injection fluids, failing to measure with a gauge and record injection and annulus pressure of its well, and failing to retain records of all monitoring information.
