On July 31, 2019, EPA Region 10 settled with Farma Tech International (1546 Boalch Avenue Northwest, Suite 30, North Bend, Washington 98045) for a violation of FIFRA Farma Tech International, failed to file the Pesticide Report for Pesticide-Producing and Device-Producing Establishments (EPA Form 3540-16) by the reporting deadline twice within five years. Reports are due by March 1 of each year. Farma Tech International failed to timely submit reports for calendar years 2018 and 2015. The company agreed to pay a penalty of $300.00.
