On June 11, 2019, EPA conducted an onsite CAA 40 CFR Part 68 and Section 112(r) compliance evaluation of the facility. Based on the PHA conducted on September 27, 2018, Respondent failed to promptly address the team's findings and recommendations in a timely manner, in violation of 40 CFR 68.67(e). Based on the documentation provided, Respondent failed to document a record that each employee involved in operating a process had received and understood the training required, in violation of 40 CFR 68.71(c).
