From August 6-10, 2018, EPA Region 6 conducted a CAA 112(r) and 40 CFR Part 68 inspection of the Facility. During EPA's inspection of the Facility, EPA identified areas of concern relating to Respondent's implementation of, and compliance with, the requirements of CAA 112(r) and 40 CFR Part 68 for Process Hazard Analysis, operating procedures, mechanical integrity, incident investigation, and emergency equipment inspection and testing.
