From October 24-26, 2017, EPA Region 6 conducted an on-site CAA 112(r) and 40 CFR Part 68 inspection of the Facility. During the inspection, EPA determined that there was a failure to follow operating procedures during temporary operations and normal shutdown, resulting in a chlorine release on April 13, 2016.
