## Summary

On April 1, 2019, EPA inspected the Kohala Coast Concrete and Precast facility to evaluate Respondent's compliance with the requirements of Sections 301 and 402(p) f the CWA and regulations promulgated under 40 C.F.R. 122.26. The EPA inspector observed gaps between the concrete-block barrier that could discharge pollutant in stormwater from the storage area during a storm event; aggregate stockpiles in the concrete production area that did not have cover or complete containment; an above ground diesel storage tank in the storage area which had no markings on the tank to indicate its storage capacity; an accumulation of miscellaneous debris, including lead-acid batteries, stored without cover or containment in the storage area; a concrete washout basin and stockpiles of hardened concrete waste material on the ground in the storage area and concrete wash water that had breached the concrete wash out basin and appeared to be traveling towards the southwestern concrete-block barrier which contained a gap. Within 30 calendar days after Respondent has fully completed and implemented the actions required by Section IV of this Administrative Order, Respondent shall submit to the EPA a final report that includes a description, including photographs, and timeline of all actions taken by Respondent to achieve compliance with this order and the CWA.
