Respondent failed to minimize pollutants in stormwater, promptly remove offsite sediment accumulations and review adequacy of controls, failed to properly select, install and/or maintain best management practices, and failed to document adequate inspections and to take appropriate corrective action for deficiencies noted in self-inspection reports.
