A post incident root cause analysis investigation was conducted by Respondent regarding a release incident and fire on May 23, 2019. It was determined that the best-known cause or root cause of the event was operator error and failure to follow established procedures, which is a violation of 40 CFR 68.69(a)(1)(vii).
