On September 16, 2019, the facility experienced a release of ammonia. The release of ammonia was due to a standard operating procedure not being adequately executed. Respondent?s failure to develop or implement written procedures that provide clear instructions for safely conducting activities involved in each covered process, consistent with the process safety information, constituted a violation of 40 CFR 68.69(a)(1)(vii).
