Facility did not repair part within 30-days of issue, did not maintain documentation of walkthrough inspections, did not maintain documentation of release detection requirements, failed to report a suspected release within 24 hrs and failed to designate a class A operator. Facility was sent a FC, paid the penalty, came into compliance and the FC was closed.
