Respondent failed to submit an accurately completed Pesticide Report for Pesticide-Producing and Device-Producing Establishments (EPA Form 3540-16), for the 2017, 2018, and 2019 calendar years. Also failed to maintain accurate production records required under Section 8 of FIFRA. Respondent submitted revised 3540-16 forms and corrected their production records in submittal from 5/19.
