On September 14, 2021, EPA Region 6 issued a Final Administrative Order (AO) under its authority pursuant to the Safe Drinking Water Act (SDWA) to Company Name located in Osage Co., Oklahoma. The AO was issued in response to violations of: 40 C.F.R. ?? 147.2903, 147.2905, 147.2907, 147.2910, and 147.2925. The proposed AO requires the Respondent to submit required annual operations report for an injection well for the period of Month Year through Month Year. The Respondent must respond to EPA Region 6 within 30 days of receiving the proposed AO.
