On August 9, 2021, EPA Region 6 issued a Proposed Administrative Order under its authority pursuant to the Safe Drinking Water Act (SDWA) to Jim H. and Anne E. Heslet located in Osage Co., Oklahoma. The proposed AO was issued in response to violations of: 40 C.F.R. ?? 147.2903, 147.2907, 147.2918, and 147.2928. The AO requires the Respondent to submit required annual operations report for an injection well for the period of April 2019 through March 2020. The Respondent must respond to EPA Region 6 within 30 days of receiving the Final AO.
