Recently, EPA was informed by the Arkansas Department of Health (ADH) that the Respondent failed to provide the CCR to its customers or the notification was not documented for the calendar years 2016, 2017, 2018, and 2019. Therefore, EPA is now ordering the Respondent to provide the Consumer Confidence Report to its customers, EPA, and ADH.
