Yucca Dental Center was issued an AO for failure to submit a One-Time Compliance Report as required by 40 CFR Part 441.50(a)(1). The AO was issued 08/30/2021 requiring a response within 30 days. AO was emailed 08/31/2021. Dr. White retired in 2018 and the person that took over this facility has since moved their practice to a different location.
