On August 17, 2021, there was an incident at the Facility that resulted in an accidental release. Pursuant to Section 114 of the CAA, the EPA requested on November 16, 2021, and Respondent provided, documentation and information concerning the Incident and Respondent's compliance with Section 112(r) of the CAA. EPA identified violations for Process Hazard Analysis, 68.67(a), (c)(3)-(5), and (c)(7), and Mechanical Integrity- Inspection and Testing, 68.73(d)(3), (e) and (f)(2).
