EPA alleged that KOG violated NESHAP 6S because it failed to: comply with the provisions of NESHAP 6S until after it installed the five new day tank glass furnaces; install certain emissions control equipment; conduct required performance testing to demonstrate compliance with NESHAP 6S's emissions limitations; perform required monitoring any time the affected furnace is producing glass that is charged with one or more of the glass manufacturing metal HAP; and is operating the Kokomo plant without a Title V permit as required by NESHAP 6S.
This ACO requires KOG to: complete installation of a baghouse/bag leak detection system at the common exhaust from KOG?s five new day tank glass furnaces; monitor performance of the day tank furnace baghouse as required by NESHAP 6S; conduct inspections of the day tank furnace baghouse as required by NESHAP 6S; maintain records as required by NESHAP 6S; maintain compliance with the NESHAP 6S emission limits; and submit a Title V permit application to the State of Indiana. These measures and other measures already taken by KOG, address the violations set forth in the FOV issued to KOG on April 8, 2016.
