This was part of national case. An EPA General Duty Clause inspection revealed that the Denver Univar facility failed to follow safe operating practices including formal tank inspections, failed to maintain formal records indicating monthly and annual tank inspections, failed to label an Extremely Hazardous Substance Tank, failed to provide documentation that monthly inspections of fixed piping, valves and flexible hose were performed, and failed to ensure annual pressure testing of flexible hoses.
