The facility failed to do the following: (1) install overfill prevention equipment, (2) maintain any documentation of compliance with release detection requirements, (3) periodic testing spill buckets, (4) periodic inspection of overfill prevention equipment, (5) periodic walkthrough inspection every 30 days, (6) annual test of release detection components, (7) designate Class C operator, and (8) maintain a list of designated operators.
