An offsite record review pursuant to EPCRA section 313 was conducted on Flotek Chemistry LLC for the most recent 5-year time period (2017-2021). During the investigation it was discovered the facility had filed Form R's late for RY's 2018 and 2019. A proposed settlement was offered to Flotek of which they accepted. However, they did ask for 4 consecutive quarterly payments for which EPA accepted. Payments shall begin on or before 6/19/2023 in the amounts of $7,043.75.
