The facility failed the following: (1) maintain documentation of compliance with release detection requirements, (2) periodic testing of containment sumps and spill prevention equipment, (3) periodic inspection of overfill prevention equipment, (4) periodic walkthrough inspection every 30 days or annual walkthrough inspection, (5) annual test of components of release detection method, (6) designate each individual who meets definition of class C operator and (7) maintain a list of designated operators. Resolved with a field citation.
