PCI violated the General Duty Clause of Clean Air Act 112(r) by failing to conduct process hazard reviews and failing to maintain a safe facility (failure to separate incompatible chemicals and failing to post NFPA diamonds on doors where flammable chemicals were stored). EPA issued an administrative order on consent in which PCI will remedy these violations and report its corrective actions to EPA.
