SmithCo Manufacturing, Inc. failed to meet numerous regulatory requirements for large quantity generators of hazardous waste. Specifically, the facility failed to keep hazardous waste containers closed, failed to perform an adequate hazardous waste determination, failed to properly train its employees, failed to inspect HW storage areas, failed to prepare a complete contingency plan, failed to coordinate with local emergency responders, failed to notify EPA of its current generator status and failure to file the required biennial reports.
