The facility failed to perform the following: periodic testing of spill prevention equipment; periodically inspect overfill prevention equipment; conduct either periodic walkthrough inspection every thirty days or annual walkthrough inspection; annually test release detection components; annual line tightness test or monthly monitoring of pressurized piping; designate each individual who meets definition of Class C operator; and maintain a list of designated operators.
