La Salle failed to maintain a proper hazard assessment, compile and maintain the necessary process safety information, conduct a proper process hazard analysis, develop all necessary operating procedures, provide refresher training to each employee, develop procedures to maintain process equipment and ensure employees were trained to maintain the equipment, conduct any compliance audits, ensure employees participated in the Risk Management Plan (RMP), and update the RMP as necessary.
This CAFO requires La Salle to pay a penalty of $25,622 and resolves the FOV issued to La Salle on June 29, 2021.
