This case addresses the following violations: 1) Failure to label a shop rag container with the words ?Excluded Solvent Contaminated Wipes?; 2) Failure to mark containers of Hazardous Waste prepared for transport with applicable EPA waste codes; 3) Failure to keep containers of Hazardous Waste closed; 4) Failure to label Hazardous Waste with an indication of the nature of the hazard; 5) Failure to mark containers of Hazardous Waste with an accumulation start date; 6) Failure to make arrangements with local emergency authorities; 7) Failure to familiarize coordinating agencies with facility layout, waste types, access points, evacuation routes, and likely casualty types; 8) Failure to maintain records documenting arrangements with response agencies; and 9) Failure to post emergency coordinator?s name and phone number, fire department number, and location of fire extinguishers and spill control equipment next to the phone.
