This case addresses the following violations: 1) failure to label a HWAC with the words ?hazardous waste?; 2) failure to label a HWAC with an indication of the nature of the hazard; 3) failure to mark a HWAC with an accumulation start date; 4) storage of hazardous waste for longer than 90 days without a RCRA permit; 5) failure to conduct weekly inspections of a HWCA; 6) failure to include a map of the Quick Reference Guide of the facility in relation to businesses, schools, and residential areas to understand how to best get to the facility and how to evacuate citizens and workers; 7) failure to submit the revised RCRA Contingency Plan to local emergency response agencies; 8) failure to prepare a written description of the type and amount of introductory and continuing training for personnel who handle hazardous waste; and 9) failure to mark accumulation start dates or otherwise track the accumulation for two universal waste accumulation containers.
