This case addresses the following violations: 1) Failure to mark three hazardous waste containers with accumulation start dates; 2) Failure to designate an emergency coordinator; 3) Failure to maintain an adequate contingency plan; 4) Failure to prepare and submit a Quick Reference Guide; and 5) Failure to conduct adequate weekly inspections.
