Respondent failed to make a hazardous waste determination on one of its waste streams. Additionally, the facility failed to properly manage their Universal Waste Lamps pursuant to the requirements set forth for a small quantity handler of universal waste. Lastly, the Respondent failed to comply with several of the RCRA safe harbor provisions. Therefore, the Respondent was operating as a transfer, storage, or disposal facility without a required permit.
