This Complaint with Notice of Opportunity for Hearing is issued to L & L Implement Company for failure to close or retrofit their Class V Well in a manner that would keep contaminants from entering the USDW. Respondent is also in violation by owning, operating or maintaining a Class V disposal facility which, through injection activity, allows the movement of fluid containing any contaminant into underground sources of drinking water.
