NPDES Permit ID-002594-1 ( Permit ) was issued to the Respondent on July 29, 1999. The Permit specified the conditions under which the Respondent may discharge treated effluent from the facility. The effective dates of the Permit were August 31, 1999, to August 31, 2004. Respondent failed to apply for a new permit as required under 40 C.F.R. Part 122.21(d)(1). Due to Respondent's failure to re-submit an application, the Permit was neither administratively extended nor re-issued.
