EPA conducted a file review of the facility and discovered that the facility had not submitted reports, retained and submitted fuel supplier certifications for all fuel delivered, recorded and maintained the amount of fuel used, and failed to identify deviations in their annual title V certification pursuant to NSPS Subpart Dc regulations.
Facility to pay a penalty of $62,250 in quarterly payments over a two year period.
