During the course of a November 29, 2005 compliance evaluation inspection of the Facility, EPA identified numberous violations of the requirements and proviisons of RCRA Subtitle Ca and of the federally-authorized Viriginai Hazardous Waste Management Regulations, including: (a) operationg a hazardous waste storage facility without a permit or interim status; (b) failing to keep containers of hazardous waste closed during storage; (c) failing to perform weekly hzardous waste container storage area inspections; (d)failing to amend the Faciltiy contingency plan; (e) failiing toensure that Facility personnel successfully completed required initial and annual refresher taining to ensure Facility compliance with applicable hazardous waste regulatory requirements; (f) failing to keep and maintain reqquired records documenting there required training given to, and compled by, Facility personnel; and (g)operating a thermal treatment unit at the Facility without a permit or interim status. Many of the identified violations are repeat violations at this Facility, as an EPA review of Virginia Department of Environmental Quality ( VaDEQ ) records and correspondence revealed that VaDEQ had previoiusly cited Electroplate-Rite for many of the same violations
