## Summary

On March 25, 2005, the Oregon Department of Agriculture (ODA) conducted a producer establishment inspection of Mt. Hood Chemical Corporation.  The ODA inspection report was sent to EPA Region 10 where it was reviewed for violations.  Upon review it was found that 13 of the 14 pesticide products inspected by ODA were in violation of the Federal Insecticide, Fungicide and Rodenticide Act.  The violations included labels with missing statements, out of data labeling, labeling that was pieced together from a combination of out of date and updated labels and some directions that did not match the EPA accepted labels.  The 11 products that are included in the Stop Sale Use or Removal Order were the products with the worst violations.  These included out of date first aid statements, altered environmental hazard statements and altered and/or out of date directions for use.  To ensure human safety and that the environment EPA has been issued this stop sale in order to curb the distribution of product with improper directions for use, first aid and environmental hazard statements.  The SSUROs contains the specific violations on each label and a cover letter with an information request was also sent to help determine who is at fault for the multiple FIFRA violations.
