AO issued for violation of CAFO permit and was ordered to do the following:
1) Prevent any further contamination of down gradient water wells.
2) Remediate contaminated water wells down gradient from the facility.
3) Dispose of animal wastewater in a manner not to exceed the nutrient uptake of the crop coverage or planned crop planting.
ODAFF issued a permit on 04/30/2015.
