This administrative order is issued to the Six K Incorporated Water System for failure to monitor quarterly for total coliform; failure to collect at least 4 repeat total coliform samples after a total coliform positive sample; failure to collect at least five routine total coliform samples after a total coliform positive sample in the preceding month; failure to monitor for nitrate; failure to conduct poublic notice; failure to report total coliform monitoring violations to EPA; and failure to report National Primary Drinking Water Regulation violations to EPA.
