Violations include: failure to collect residual disinfectin samples; collect and analyze monthly, routine samples or total coliform; collect and analyze quarterly samples for disinfection by products; provide annual consumer confidence reports to its customers; provide public notice for non-sampling and non-testing of total coliforms,total trihalomethanes and haloacetic acit (five); and failure to obtain a qualified operator.
