This Administrative Order is issued to the Thunder Child Treatment Center for failure to monitor for total coliform bacteria, failure to collect a set of repeat samples following a total coliform positive routine sample, failure to collect 5 routine samples after a total coliform positive sample in the preceding month, failure to notify the public of the violations, and failure to notify EPA of the violations.
