This Complaint with Notice of Opportunity for Hearing is issued to Pendragon Energy Partners for injecting into a well lacking mechanical integrity; failure to have a 1/2 inch fitting on the well's annulus to allow for placement of a pressure gauge; and failure to timely submit the 2006 annual monitoring report to EPA. This well is located on the Uintah and Ouray Indian Reservation within the State of Utah.
