Respondent failed to include, either within the rental contract or as an attachment to the contract, a lead warning statement, a statement diclosing the presence of known lead-based paint (LBP) and/or LBP hazards, a list of any records or reports available to the lessor that pertain to LBP and/or LBP hazard information, a statement by the lessees affirming receipt of the information required by 40 C.F.R. 745.113(b)(2) and (b)(3), and the signatures of the lessor and the lessees certifying to the accuracy of their statements and the dates of such signature before the lessees were obligated under the rental contracts.
